29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e): a) On or about April 1, 2011, employees were not protected from fall hazards when working in the basket of the aerial lift. Employees were wearing full body harnesses, but were not connected to any anchor points. Abatement note: All platform occupants must wear a fall restraint device connected to a lanyard anchor point.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) On or about April 1, 2011, employees were exposed to fall hazards of approximately 40 feet while working on a roof with no fall protection system in place. b) On or about April 1, 2011, employees were exposed to falls when exiting the basket of the aerial lift to access the roof. No fall protection system was in place between the lift and the roof.
