29 CFR 1926.95(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) Foam Tech Unlimited, Inc @ 41024 Lamborn Mesa Rd., Paonia, CO 81428: On or before 3/30/11, the employer did not ensure that employees were equipped with protective clothing. Employees are exposed to chemicals which include, but not limited to, aromatic isocyanates during spray foam application. The employer provided second-hand street clothing for use during the application process. This condition exposed employees to chemicals which may sensitize via dermal exposure.

Employer supplied respirators were stored in a shelf in the back of the work vehicle and were, therefore, capable of becoming damaged and/or contaminated prior to employee use while conducting spray foam application.
