29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service until repairs and tests necessary to render the equipment safe were made: 1) On March 29, 2011, and at prior times, the electrical cord on the 'Bali' blind cutter was damaged, in that the jacket was pulled away from the strain relief on the frame of the cutter.
