OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) Cloverdale Memorial Garden; Terrace Lawn grounds; on or about March 22, 2011, and at time prior thereto, employees for digging and preparing gravesites, approximately 3 feet wide and 6 feet deep. Employees were exposed to cave in hazards without the benefit of a competent person or cave in shielding.

Cloverdale Memorial Garden shop; on or about March 22, 2011, and at time prior thereto, employees worked around live electrical conductors (120-240 volts) in several unidentified, uncovered, electrical junction boxes on the east wall of the shop.
