29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage. Sharp corners were not avoided: a) On or about April 18, 2011 and at times prior, Ball Construction Services, Inc. as controlling employer, did not ensure that subcontractor employees were protected from injury in that, several flexible cords and cables connected to the temporary power distribution center on the first floor, were hung with wire and wrapped around sharp edges of steel frames which could damage the insulation of the cord and cables.

29 CFR 1926.501(c): When employees were exposed to falling objects, the employer did not implement one of the following measures; 1) Erect toeboards, screens, or guardrail systems to prevent objects from falling from higher levels; or, 2) Erect a canopy structure and keep potential fall objects far enough from the edge of the higher level so that those objects would not go over the edge if they were accidentally displaced; or, 3) Barricade the area to which objects could fall, prohibit employees from entering the barricaded area, and keep objects that may fall far enough away from the edge of a higher level so that those objects would not go over the edge if they were accidentally displaced: a) On or about April 18, 2011 and at times prior, Ball Construction Services, LLC as controlling/creating/correcting/exposing employer, did not ensure that employees as well as subcontractor employees working at the south building, were protected from falling objects in that, toe boards or other means, were not used in all locations where materials and tools could fall from upper levels.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material to the top edge of each pan: a) On or about April 18, 2011, Ball Construction Services, LLC as controlling/exposing employer, did not ensure that employees and subcontractor employees were protected from injury in that, stair pans on the 4th floor and the landing below the 4th floor had a raised edge that created a tripping hazard. The pans were not filled with solid material or protected by other means.
