(a) On or about March 8, 2011, and at times prior thereto, the employer did not provide and/or enforce the use of fall protection for employees involved in sheathing activities and exposed to fall hazards. (b) On or about March 8, 2011, and at times prior thereto, a hole cut in the floor for the construciton of stairs to the basement level was not protected to prevent employees from tripping, falling and/or stepping into the hole. C. WELLS LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311578553, CITATION NUMBER 1, ITEM 1, AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 24, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 4846 HAWTHORNE ROAD, CHUBBUCK, ID 83202.
