29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:\r\n\r\n(a) Colorado Container Corporation at 4221 Monaco St, Denver, CO 80216: On and before April 14, 2011, the employer did not conduct a periodic inspection at least annually of the Lockout/Tagout (LOTO) procedures used by maintenance personnel. This condition exposed employees to the hazards of compressed air, 3-phase 480 volts, hydraulic energy, as well as other sources of stored energy. Maintenance personnel perform LOTO procedures on the following equipment (not all equipment is listed below):\r\n\r\n(1) Langston Flexo Folder Gluer\r\n(2) Hycorr Rotary Die Cutter\r\n(3) McKinkley Rotary Slotter\r\n(4) Signode unit banders\r\n(5) semiautomatic stitcher\r\n(6) tapers\r\n(7) slitters\r\n

This condition exposed employees to hazards by contacting the chuck.\r\n(b) Colorado Container Corporation at 4221 Monaco St, Denver, CO 80216: On and before April 14, 2011, guarding was not provided to protect machine shop employees from hazards created by the \\r\\nthe rotating parts of the Rutland Tool & Supply Co. milling machine, serial number 5088.\r\n(c) Colorado Container Corporation at 4221 Monaco St, Denver, CO 80216: On and before April 14, 2011, guarding was not provided to protect machine shop employees from hazards created by the \\r\\nthe rotating parts of the Delta milling machine, serial number 115-1921.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 psi:\r\n(a) Colorado Container Corporation at 4221 Monaco St, Denver, CO 80216: On and before April 14, 2011, the employer did not ensure the compressed air nozzle used for cleaning off the Signode Unit Bander, machine number 2089, was reduced to less than 30 psi. The compressed air line was pressurized at approximately 80 psi. This condition exposed employees using the air hose to the hazard of an embolism in the blood stream.\r\n
