29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) On or about April 20, 2011 and at times prior, the employer did not establish and implement a respiratory protection program when employees were using half face, tight-fitting respirators at the site. Deficiencies observed included: 1. No written program. 2. No procedures for selecting proper respirators. 3. Fit testing had not been completed for employees wearing tight-fitting respirators. 4. Procedures were not developed for proper use of respirators. 5. Respirators at the site were unsanitary and in poor repair. Procedures and schedules had not been developed for cleaning, disinfecting, inspecting, repairing, discarding and storing respirators. 6. Employees had not been trained on proper use of respirators and respiratory hazards. 7. Procedures were not developed to evaluate the program.
