29 CFR 1926.200(g)(2):  The employer did not ensure that all traffic control signs or devices used for protection of construction workers conformed to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD) Millenium Edittion, December 2000:         a)  Mainlining Service, Inc. at 1900 S. Corona St., Denver, CO 80210:  On or before April 26, 2011, the employer did not ensure that advanced warning signs conformed to the MUTCD in that employees were working in two work zones involving two trenches without adequate signage.  The employer did not have advanced warning signs on the south side (northbound on S. Corona St.).  In addition, the only advanced warning sign for the West side (eastbound on Jewell Ave.) was 5 blocks away on Pearl St.  Only one sign was in place on the north and east sides of the intersection.  Conditions exposed employees to struck-by hazards.    Abatement note:  Abatement certification is required for this item (see enclosed ,"Sample Abatement-Certification Letter").    Abatement note:  The MUTCD Section 6B.01 provides that the control of road users through a temporary traffic control zone shall be an essential part of high construction, utility work, maintenance operations, and incident management.  Sections 6C.03 and 6C.04 discuss the requirements associated with advance warning areas, providing that the advanced warning area is that section of the road where users are informed about the upcoming work zone or incident area.  Typical Application 6H-21 illustrates the signage and barriers for a lane closure on the near side of an intersection, and is applicable to the trench on Jewell Ave.  Typical Application 6H-22 Illustrates the right lane closure on the far side of the intersection, and is applicable to the trench on S. Corona St.

29 CFR 1926.202:  The employer did not ensure that all barricades for protection of construction workers conformed to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD) Millenium Edition, December 2000:         a)  Mainlining Service, Inc. at 1900 S. Corona St., Denver, CO 80210:  On or before April 26, 2011, the employer did not ensure that the barricades in use conformed to Part VI of the MUTCD in that no tapers were used to direct traffic away from two work zones involving two trenches on two different lanes at this intersection.   Conditions exposed employees to struck-by hazards.    Abatement note:  Abatement certification is required for this item (see enclosed ,"Sample Abatement-Certification Letter").    Abatement note:  The MUTCD Section 6C.08 discusses the requirements associated with temporary traffic control zones, including the use of tapers with channelizing devices to move traffic out of or into normal paths.  Section 6F.55 desribes the use of cones and other barricades to serve as channelizing devices.  Typical Application 6H-21 illustrates the signage and barriers for a lane closure on the near side of an intersection, and is applicable to the trench on Jewell Ave.  Typical Application 6H-22 Illustrates the right lane closure on the far side of the intersection, and is applicable to the trench on S.
