29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word \\\"HOLE\\\" or \\\"COVER\\\" to provide warning of the hazard:\\r\\n(a) Hayhurst Construction, Inc. at Hayhurst Construction, Inc: On and before May 4, 2011, the exposing employer did not ensure that employees were protected from falling into holes, in that several holes on the east side of the building were covered with unmarked 1/2 inch sheets of plywood. The openings of the holes were approximately 2 to 3 feet and ranged from 2 feet to 4 feet in depth. This condition exposed employees to the hazards of falling into the unmarked hole.\\r\\n

29 CFR 1926.1053(b)(6): Ladders were not secured to prevent accidental displacement when used on surfaces which were not stable and level: (a) Hayhurst Construction, Inc. at Hayhurst Construction, Inc: On and before May 4, 2011, the exposing employer did not ensure that ladders were secured when used on unstable surfaces, in that an employee was injured when the ladder he was working on slipped causing the employee to fall to the ground. The base of the extension ladder was placed on top of a sheet of plywood. The base of the ladder was not secured to the plywood, nor was the plywood secured to the ground. This condition exposed the employee to a fall of approximately 14 feet to the ground below.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:\r\n(a) Hayhurst Construction, Inc. at Hayhurst Construction, Inc: On and before May 4, 2011, the exposing employer did not provide a training program for each employee using ladders to train each employee in the procedures to be followed to minimize hazards related to ladders. This condition exposed the employees to fall hazards approximately 20 feet.\r\nAbatement Note: The employer shall ensure that each employee has been trained by a competent person in the following areas, as applicable:\r\n(a) The nature of fall hazards in the work area;\r\n(b) The correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used;\r\n(c) The proper construction, use, placement, and care in handling of all stairways and ladders;\r\n(d) The maximum intended load-carrying capacities of ladders; and\r\n(e) The standards contained in this subpart.\r\n\r\n
