29 CFR 1926.451(g)(1): Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level: (a) W.H.K. Masonry, Inc. at 310 S. 8th Ave, Frisco, CO 80443: On and before May 5, 2011, the exposing employer did not ensure that employees were protected from a fall from elevation, in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffolding. The scaffold on the east side of the building lacked midrails and endrails on both the 4th and 5th levels of the scaffold. Employees worked on the 4th and 5th levels of the scaffold at this location. This condition exposed employees to falls of approximately 33 feet. (b) W.H.K. Masonry, Inc. at 310 S. 8th Ave, Frisco, CO 80443: On and before May 5, 2011, the exposing employer did not ensure that employees were protected from a fall from elevation, in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffolding. The scaffold on the east side of the building lacked endrails on both the 4th and 5th levels of the scaffold. One employee was standing at the end of the 4th level scaffold platform handing up a scaffold component to another employee standing at the end on the 5th level. This condition exposed employees to falls of approximately 33 feet. (c) W.H.K. Masonry, Inc. at 310 S. 8th Ave, Frisco, CO 80443: On and before May 5, 2011, the exposing employer did not ensure that employees were protected from a fall from elevation, in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffolding. The scaffold on the north side of the building lacked midrails and endrails on the 4th level of the scaffold. Employees worked on the 4th level of the scaffold at this location. This condition exposed employees to falls of approximately 26 feet. (d) W.H.K. Masonry, Inc. at 310 S. 8th Ave, Frisco, CO 80443: On and before May 5, 2011, the exposing employer did not ensure that employees were protected from a fall from elevation, in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffolding. The scaffold on the west side of the building lacked a midrail on the 4th level of the scaffold where employees accessed from inside the building. This condition exposed employees to falls of approximately 26 feet.
