29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.    (a)  Panera Bread Fresh Dough Facility @ 6360 E. 58th Avenue, Unit C @ Commerce City, CO 80020: On and before March 28, 2011, the employer did not ensure that the listing and labeling of electrical equipment was followed, in that, a flexible power cable that was suspended from the ceiling was not tied into knots to prevent the plug end from hitting the floor. The employer tied the flexible cord into knots to shorten the length. The employer exposed employees to shock and burn hazards as a result of tying the flexible cable into knots which damages the inner conductors within the cable.

(a)  Panera Bread Fresh Dough Facility @ 6360 E. 58th Avenue, Unit C, Commerce City, CO 80020: On and before March 28th , 2011, the employer did not ensure that the flexible cord connection between the bagel divider/former equipment had connective devices and fittings so that tension would not be transmitted to joints or terminal screws, which allowed the inner conductors to become exposed. This condition exposed employees to shock, burn, and electrocution hazards.
