Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to a potential crushing and/or amputation hazard: (a) At Adams Thermal Systems located in Canton, SD, where, on or about 03/24/11, an employee was exposed to a potential crushing and/or amputation hazard while operating the Niagra N120-8-10 press brake, serial #43456. Among other methods, one feasible and acceptable abatement method to correct this hazard is to adhere to the requirments of ANSI B11.2-1995 Hydraulic Power Presses - Safety Requirements for Construction, Care, and Use: Section 6.3.3 Control Reliabilty: paragraphs 1, 2, and 3.
