1926.405(b)(2): All pull boxes, junction boxes, and fittings were not equipped with covers: (a) First Call Electric & Fire, Inc. @ 2010 14th St., Boulder, CO 80302: On and before 5/9/11, the employer did not ensure that a temporary electric panel, which was live with 120 Volts, was equipped with a dead front panel. This exposed employees and sub-contracted employees to shock and/or electrocution hazards.
