Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrailsystems, safety net systems, or personal fall arrest systems. a) On or about April 27, 2011 and at times prior thereto, employees involved in the installation of a mechanical unit on a roof structure were exposed to the potential fall hazard to the ground surface below. b) On or about April 27, 2011, and at times prior thereto, an employee was exposed to a 9 foot all hazard while working on top of a mechanical unit.
