29 CFR 1926.304(i)(1):  Each circular hand-fed rip saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:    a) On the jobsite: On or about May 12, 2011, and at times prior thereto, the Porter-Cable portable table saw being used to cut trim was did not have a blade guard. 29 CFR 1926.62(d)(2)(v)(B):  Until the employer performed an employee exposure assessment as required under 29 CFR 1926.62(d) and determined actual employee exposure, the employer did not provide employees performing the tasks described in 29 CFR 1926.62(d)(2)(i), (ii), (iii), and (iv) with appropriate personal protective clothing and equipment in accordance with 29 CFR 1926.62(g):  	  a) On the jobsite: On or about May 12, 2011, and at times prior thereto, employees engaged in replacing windows with trim painted in lead-based paint were not provided with appropriate personal protective clothing and equipment  including but not limited to coveralls or similar full-body work clothing, gloves, hats, and disposable shoe coverlets  until an exposure assessment had been performed.
