CFR 1926.20(b) (2):  Employer did not provide programs for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  	(a)	Statewide Renovators, 4550 East Jewell Avenue, Denver, CO 80222:  On and before May 17, 2011, as the controlling employer who booked the job, dictated the jobs start and end dates along with the scope of work, did not ensure through frequent and regular inspections that adequate fall protective measures were in place.  Specifically, two sub contractor employees were witnessed working off an approximately 40 foot three story flat roof without adequate fall protective measures in place.  One employee was witnessed leaning over the roofs edge to raise up electrical cords and other material and tools with no fall protection.  Condition exposed employees to fall hazards of approximately 40 feet.    Abatement Note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
