The railing shall be provided with a toeboard wherever, beneath the open sides:    (a) An employee was working on a work platform that did not have an adequate guardrail system on the north side of the building located at 500 Rainbow Parkway in Brookings, South Dakota.

(b) Employees were exposed to an inadequately guarded chuck on a drill press in the four-by-four drilling area at 500 Rainbow Parkway in Brookings, South Dakota.
