29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) On or about May 20, 2011, empoloyees were exposed to cave-in hazards in that, a competent person had not conducted an inspection of the excavation at the site to determine soil classification and what protective system would be necessary.
