29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and personal fall arrest system, or warning line system and safety monitoring system:    (a)Pacific General Roofs, LLC @ 1011 16th St., Boulder, CO 80302: On and before May 14th, 2011, the exposing employer did not ensure their employees were protected from injury, in that employees engaged in re-roofing operations on the roof of the Chi Omega sorority were exposed to falls of approximately 25'. Employees were engaged in roofing on a flat roof that transitioned into a 15 in 12 roof. Employees did not utilize a fall protection system while roofing work was bieng conducted. The condition exposed employees to a 25 foot fall hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)Pacific General Roofs, LLC @ 1011 16th St., Boulder, CO 80302: On and be May 14th, 2011, the exposing employer did not ensure that employees involved in re-roofing activites on the Chi Omega sorority house,  working on a 15 in 12 pitched roof were protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. The  condition exposed employees to a 25' fall hazard.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder    (a) Pacific General Roofs, LLC @ 1011 16th St., Boulder, CO 80203: On and before May 14th, 2011, the exposing employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface.  Employees were using an extension-ladder for access to the roof off the rear fire escape on the Chi Omega sorority house. Employees were exposed to a fall of approximately 20 feet off the fire escape while attempting to use the ladder.
