29 CFR 1926.652(a)(1):     Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a.  On or about May 26, 2011, at the above addressed jobsite, an employee was installing a sewer line in an excavation measured at seven (7) feet in depth.  The excavation was not provided with cave in protection, thereby exposing the employee to cave in hazards.
