29 CFR 1904.7(b)(3):  How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.    a.	On or about May 25, 2011, the employer did not enter correctly the following workplace injuries or illnesses on the OSHA Form 300 or its equivalent for the calendar year 2010:  For case number 10-01 which was an injury due to a twisted knee with 39 days away from work, the employer entered a check mark in column I and did not enter days away from work.

29 CFR 1904.32(b)(1):  How extensively do I have to review the OSHA 300 Log entries at the end of the year? You must review the entries as extensively as necessary to make sure that they are complete and correct.    a.	On or about May 25, 2011, the employer did not review the entries as extensively as necessary to make sure that they were complete and correct on the OSHA Form 300 or its equivalent for the calendar year 2008:  Case numbers 04-07 were for another business location; the remaining five injuries did not have check marks in column J; there was no check mark entered for case number 08 in column M1; and the totals were not entered correctly.

29 CFR 1910.132(d)(2):  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a.	At the workplace, the employer did not verify that the required workplace hazard assessment for personal protective equipment had been performed through a written certification for employees performing tasks such as, but not limited to, adding chemicals, cleaning tanks, and welding.
