29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    On or about May 24, 2011 an employee was laboring tools and roofing materials above his head without a hard hat.  The employee was exposed to overhead struckby hazards.    Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about May 24, 2011 employees were working on a low slope roof performing roofing work without fall protection exposing the employees to a fall hazard of 24 feet.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer did not retrain each such employee.     On or about May 24, 2011 an employee was using a harness tied to a lanyard and a rope that extended to a length that would not have provided adequate or safe fall protection exposing the employee to the hazards associated with improper use of fall protection.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
