29 CFR 1926.102(a)(1): Eye and face protective equipment were not provided when machines or operations presented potential eye or face injury:  (a)  TS Construction Inc at 8133 E 34th Ave, Denver, CO 80238:  On and before May 28, 2011, the exposing employer did not ensure employees were provided eye protection while operating pneumatic nail guns while framing a residential home. This condition exposed employees to projectile hazards to the eyes.  (b)  TS Construction Inc at 8133 E 34th Ave, Denver, CO 80238:  On and before May 28, 2011, the exposing employer did not ensure employees were provided eye protection while operating hand operated circular saws while framing a residential home. This condition exposed employees to projectile hazards to the eyes.  Abatement Note:  1926.28(a) also provides that the employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:  (a)  TS Construction Inc at 8133 E 34th Ave, Denver, CO 80238:  On and before May 28, 2011, the exposing employer did not ensure that employees were protected from injury, in that an employee installing roof sheeting on the edge of the garage roof on a residential home was not utilizing a fall protection system. The slope of the roof was 9-in-12. This condition exposed the employee to a fall hazard of 13 feet.  (b)  TS Construction Inc at 8133 E 34th Ave, Denver, CO 80238:  On and before May 28, 2011, the exposing employer did not ensure that employees were protected from injury, in that employees framing the second level of a residential home were not utilizing a fall protection system. Employees worked at the unprotected edges while framing. This condition exposed employees to a fall hazard of 14.5 feet.

29 CFR 1926.1053(b)(1):  A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet above the upper landing, and was not secured:  (a)  TS Construction Inc at 8133 E 34th Ave, Denver, CO 80238:  On and before May 28, 2011, the exposing employer did not ensure that a portable extension ladder, used to access the 2nd level of the residential home, had side rails extending 3 feet above the upper landing, and was secured. This condition exposed employees to fall hazards of approximately 14.5 feet.
