29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support: a) On or about June 3, 2011, employees were exposed to fall hazards in excess of 20 feet where the portable ladder used to gain access to the working level of the sign did not extend at least 36 inches above the working level , nor was the ladder secured.
