29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a)A Plus Roofing, 14867 Falcon at Wildgrass, Bloomfield, CO: On or about June 6th, 2011, employees installing shingles were exposed to falls up to an estimated 10-12 feet due to excessive slack in the two lifelines used with Personal Fall Arrest Systems. Abatement Note: Abatement certification is not required for this item.
