29 CFR 1910.212(a)(3)(ii):  Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:         a)  Mark VII Equipment, Inc. at 5981 Tennyson Street, Arvada, CO 80003:  On or before 6/7/11, the employer did not ensure that the following machinery was guarded at the points of operation:         1)  The Omada 125 press brake, model 12536, serial number 1250131, was not guarded around its point of operation, including the front and the back;         2)  The Omada 80 press brake, model 80, serial number 811308, was not guarded around its point of operation, including the front and the back;         3)  The Omada shear, model H3013, was not guarded behind the point of operation; and         4)  The Omada laser, model AF4000E, was not guarded around the perimeter of the material bed.    Conditions exposed employees to amputation hazards.    Abatement note:  Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter.    Abatement note:  To guard the rear of the press brakes and the shear, prevent employee access while the machines are in operation via a barrier, along with appropriate employee training and enforcement.  To guard the perimeter of the laser bed, expanded metal should be a viable option.

29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:         a)  Mark VII Equipment, Inc. at 5981 Tennyson Street, Arvada, CO 80003:  On or before 6/7/11, the employer did not develop, document, and utilize specific lockout tagout procedures for the following equipment:         1)  The Omada 125 press brake, model 12536, serial number 1250131;         2)  The Omada 80 press brake, model 80, serial number 811308;         3)  The Omada shear, model H3013; and         4)  The Omada laser, model AF4000E.    Conditions exposed employees to amputation hazards.    Abatement note:  Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter.    Abatement note:  1910.147(c)(4)(ii) provides that the procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized in the control of hazardous energy, and the means to enforce compliance.

Abatement note:  1910.147(c)(6)(ii) provides that the employer shall certify that the periodic inspections have been performed.  The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

Some injuries that were recorded were not classified as Days Away from Work (Column H), Job Transfer or Restriction (Column I), or as Other Recordable Cases (Column J).  Condition resulted in unclear reporting, classifying, and summarizing injuries.    Abatement note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter.    Abatement note:  1904.32(a)(3) provides that the employer shall certify the summary.
