29 CFR 1910.23(c)(1):     Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    The employees were unsafe accessing machinery 4 to 5 feet from ground by climbing onto machines without fall protection or safe method of access.    No additional documentation is required for this item.

29 CFR 1910.147(c)(7)(i)(A):     Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.    The authorized employees were not adequately trained in lockout/tagout procedures.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
