29 CFR 1926.405(b)(2): Outlet boxes were not provided with covers, faceplates, or fixture canopies:     a) The employer did not guard or otherwise protect subcontractor employees from accidental contact with energized outlets and switches operating at 120 volts.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric powercircuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock bydeenergizing the circuit and grounding it or by guarding it effectively by insulation or other means:    a) Subcontractor employees were exposed to electrical hazards where caprentry work was conducted within 2 feet of a service drop power line having a voltage rating of 120/240 volts.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
