29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrailsystems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. On or about May 26, 2011, at the above addressed jobsite, employees conducting siding operations from a Pump Jack Scaffold, thirteen (13\') feet eleven (11\") inches above the ground, were exposed to fall hazards due to the lack of a complete guardrail system.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, 29 CFR 1926.454 (a)(1) through (a)(5): On or about May 26, 2011, at the above addressed jobsite, an employee conducting siding operations from a pump jack scaffold was not trained to recognize the hazards associated with scaffolding. The employee was thereby exposed to the hazards associated with falls.
