29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Soto-Villa, Enrique dba Soto Siding at 634 Draw Street, Brighton, CO 80603: On or before June 4, 2011, the employer did not ensure that the basement window wells were covered when emplioyees were siding adjacent to the holes. The site included three window wells measuring 52" x 36" and 64" deep. Condition exposed employees to fall hazards.
