29 CFR 1910.95(c)(1):     The employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent:    An employee working in the blasting booth was exposed to continuous noise at 90.43% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) during the 392 minute sampling period on 8/10/2011; exposure calculations include a zero increment for the 88 minutes not sampled.     Note:  Knapp Foundry was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 31150088, citation number 1 item number 1 and was affirmed as a final order on 9/15/2008.

29 CFR 1910.134(i)(7):     Oil lubricated compressor(s) used to supply breathing air did not have a high-temperature or carbon monoxide alarm(s) or both:    Although a carbon monoxide alarm was provided, it was not in working condition because the tube which connected it to the air supply was disconnected and no calibrations were done as required by the supplier of the unit.    Note:  Knapp Foundry was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 31150088, citation number 1 item number 3d and was affirmed as a final order on 9/15/2008.

29 CFR 1910.157(g)(2):     The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    Employees were not trained in the use of fire extinguishers since 2009.  Training was not done in 2010 and 2011.    Note:  Knapp Foundry was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 31150088, citation number 1 item number 4 and was affirmed as a final order on 9/15/2008.
