29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers: The Wood Companies, 935-951 North High Street, Wood Building Columbus Ohio worksite - As of and prior to June 14, 2011, there were two twelve inch deep holes cut into the concrete walking surface that were not covered or protected from being stepped into. The field supertendent had daily exposure to this condition when accessing the vacant commercial space being used as the jobsite office.
