29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) West side of stucture, north of main entrance; On or about 6/14/11 and at times prior thereto, an employee was installing an aluminum window frame from a suspended canopy without a means of fall protection. Note: Abatement certification is NOT required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) West side of stucture, north of main entrance; On or about 6/14/11 and at times prior thereto, an employee utilized an aluminum extension ladder for access while installing an aluminum window frame from a suspended canopy without extending the ladder at least 3 feet above the canopy deck. Note: Abatement certification is NOT required for this item.
