29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On June 14, 2011 at a worksite located at 415 Ogden Ave, Clarendon Hills IL. An employee was working on a commercial building performing welding. The Miller full body harness the employer provided the employee had a tear on the leg strap. The employee was working approximately 24.2' high. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.760(a)(1): Each employee engaged in steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On June 14, 2011 at a worksite located at 415 Ogden Ave, Clarendon Hills IL. An employee was working on a commercial building screwing in sheeting. The employee was not utilizing any means of fall protection and was exposed to a fall of approximately 25.0 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
