29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      (a)	On 6/15/ 2011, and at times prior thereto, the employer could not provide documentation of the lock out / tag out procedures to follow when changing the blades on equipment such as but not limited to: debarker, Cut off saw, C Frame head rig saw, Schumer edger gang saw.  (b)	On 06/15/11, and at times prior thereto, lock out / tag out procedures for employees operating, adjusting, un jamming and cleaning equipment such as but not limited to the in feed and out feed conveyors of the cut off saw,  C Frame head rig, Schumer edger  gang saw were not developed.  (c)	On 6/15/2011 and at times prior thereto the employer could not provide documentation of the lock out / tag out procedures to follow when changing the knives/ hammers on the Hammer mill and sizing conveyors.
