29 CFR 1910.1001(j)(2)(i):     The building owner or facility owner did not determine the presence, location, and quantity of presumed asbestos-containing material (PACM) and/or other asbestos-containing material (ACM) at the work site:    (a) 5340-48 N. Kenmore Ave, Chicago, IL basement - On or about June 13, 2011, the employer did not determine the presence, location and quantity of presumed asbestos containing material on thermal system insulation (TSI) located on pipes and a boiler.     NOTE: OTHER ACM IS ASPHALT FLOORING AND VINYL FLOORING MATERIAL INSTALLED NO LATER THAN 1980 AND OTHER INSTALLED MATERIALS THAT THE OWNER WOULD HAVE IDENTIFIED AS ASBESTOS-CONTAINING IF THE OWNER HAD EXERCISED DUE DILIGENCE.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
