29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) J. Eagle Framing Co. LLC at 732 Draw Street, Brighton, CO 80601: On or before 6/4/11, the employer did not ensure that electrical equipment was installed and used in accordance with instructions, in that a metal 110 volt 4 plex receptacle outlet box was connected to a flexible cord and 220 volt plug, and plugged into the 220 volt receptacle in the temporary panel. This metal 4 plex receptacle outlet box had holes in the back, was mssing a knockout, was set on the soil, and was not listed or labelled for such use. This box was connected to three 110 volt extension cords used to power the air compressor and skilsaws. Condition exposed employees to electrical shock hazards. Abatement note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.404(b)(1)(ii): Alll 120-volt, single-phase 15- and 20-ampere receptacleoutlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, did not have approved ground-fault circuit interrupters for personnel protection: a) J. Eagle Framing Co. LLC at 732 Draw Street, Brighton, CO 80601: On or before 6/4/11, the employer did not ensure that all 120 volt circuits had approved ground fault circuit interruption. The metal 110 volt 4-plex outlet box was plugged into a 220 volt outlet and did not provide GFCI protection. Three extension cords were plugged into the metal outlet box to power the air compressor and skilsaws, and these tools were not double insulated. Conditions exposed employees to electrical shock hazards of up to 220 volts. Abatement note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

LLC at 732 Draw Street, Brighton, CO 80601: On or before 6/4/11, the employer did not ensure that the protable tools were properly grounded in the following instances: 1) The metal 4-plex outlet box that was connected to 3 extension cords was not grounded. The extension cords powered the air compressor, and other skilsaws. The metal outlet box was connected to a 220 volt outlet and was not on ground fault circuit interrupter. The tools were not double insulated; and 2) A skilsaw model H08425 Serial number HF706183 was missing its ground plug. Conditions exposed employees to electrical shock hazards of up to 220 volts.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected frpm falling: a) J. Eagle Framing Co. LLC at 732 Draw Street, Brighton, CO 80601: On or before 6/4/11, the employer did not ensure that employees were protected from falling by a guardrail system, personal fall arrest system, or a safety net in the following instances: 1) An employee was sheathing the roof while standing on a 4' x 8' OSB with a 6 in 12 pitch, with no fall protection; 2) Employees were framing the interior of the house, and the following second floor window openings were not guarded: a) One window opening on the east side measured 35" high from the floor and 6' long; and b) Two window openings on the west side measured 35" high from the floor and 5' long. Conditions exposed employees to fall hazards of up to 17'. Abatement note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
