29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    a) On or about June 8, 2011, and at times prior thereto, an employee was observed utilizing a skill saw to cut sheathing materials and was not required utilize eye and/or face protection.     b) On or about June 8, 2011, and at times prior thereto, two employees observed utilizing pneumatic nail guns on a 8 in 12 pitch roof structure were not required to utilize eye and/or face protection.

Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provisionin paragraph (b) of this section provides for an alternative fall protection measure.     a) On or about June 8, 2011, and at times prior thereto, two employees were observed sheathing an 8 in 12 pitch residential roof structure without implementing any means of fall protection and exposed to a 12 foot fall hazard.
