29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to a serious crush-by hazard that could result in injury or death by workiing on an Oshkosh snow sweeper without the hood access safety pin being installed.    a) VM #2 Bldg 1100:  on June 3, 2011, and times prior thereto, vehicle mechanics were working on an Oshkosh snow sweeper with out the hood safety pin installed in the engine hood lockout brace.    Note 1: Among other methods, one feasible and acceptable means of abatement would be for the employer to follow the vehicle technical manual, TO 36C18-44-1, dated September 1, 1991  "Operation and Operator Maintenance Instructions" that requires:    Chapter 3-4.6.1 Drive Engine Access-  "Warning, do not work in drive engine compartment unless hitch pin is installed in lockout strut.  If pin is not installed, lockout strut could accidently collapse causing the hood to fall.  Failure to comply could result in serious injury to personnel.     Note 2: Abatement certification is required for this item.

In the case of an accidental fall that rendered the operator unconscious, there would be no guarding mechanism to prevent the operator from being carried into the crusher/compactor.
