You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.\r\n\r\na) a) At the establishment, column \"F\" of the OSHA-300 for 2010 did not include all the information required for two of the recorded injuries and illnesses. Column \"F\" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made the employee ill. The two recorded injuries occurred on January 14, 2010 and on May 6, 2010.
