29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:    (a)  On or about June 17, 2011, and at times prior, the employer did not ensure that the scaffold caster wheels adequately locked to prevent movement of the scaffold in that the employee working from the scaffold could jog the scaffold to move towards the west while working on a roofing project at the Lake Elizabeth Pavilion on the River Walk of Pueblo in Pueblo, Colorado.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement:    (a)  On or about June 17, 2011, and at times priore, the employer did not ensure that its employee was protected from fall hazards in that the wheels of the scaffolding did not all bear on the ground equally making the scaffod tip as the employees shifted weight while working from the scaffold on a roofing project at the Lake Elizabeth Pavilion on the River Walk of Pueblo in Pueblo, Colorado.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    (a)  On or about June 17, 2011, and at times prior, the employer did not inspect the mobile scaffold and did not identify that the caster wheels did not lock sufficiently to prevent movement of the scaffold exposing the employee to fall hazards while working from the scaffold on a roofing project at the Lake Elizabeth Pavilion on the River Walk of Pueblo in Pueblo, Colorado.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    (a)  On or about June 17, 2011, and at times prior, employees were exposed to fall hazards because the ladder they used to access the roof of the Lake Elizabeth Pavilion on the River Walk of Pueblo in Pueblo, Colorado, had dented rungs and side rails.
