29 CFR 1926.251(e)(8)(iii): Synthetic slings were not immediately removed from service when snags, punctures, tears or cuts were present: a) Hi-Ball Crane Services, Inc. at 1073 Copper Hill Ct., Louisville, CO 80027: On or before 6/18/11, the creating and correcting employer did not ensure that a Cable Tech synthetic sling, serial number 101187, was removed from service when tears or cuts were present on two places on the sling, evidenced by red thread. Framing employees were using this rigging to connect to and disconnect from loads of OSB sheathing consisting of 68 4'x 8' sheets and weighing 3,500 pounds per load, and these employees were standing in the fall zone of these loads. Conditions exposed these employees to struck-by hazards. Abatement note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

The distance to the second step was 26". Conditions exposed these employees to tripping hazards.
