OSH ACT of 1970 Section (5)(a)(1): \r\n\r\nSection 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to failure of a main guy line resulting in a struck by hazard of a flying wire cable and possible rig collapse.\r\n\r\na) On or about April 18, 2011 and at times prior, Eastern Colorado Well Service did not ensure that employees were protected from struck by hazards. One of the main guy lines for the work over rig was not properly installed. The four fist grip clamps, used to attach the line at the anchor point, were each missing a tightening nut.\r\n\r\nAmong other methods, one feasible and acceptable abatement method to correct this hazard would be: Establish a rig up inspection list which would be used to ensure that guy lines and other rig stability issues are addressed before beginning operations.\r\n\r\n \r\n

The hand held dirt auger had been field rigged to operate with the dead man switch removed. \\r\\n
