29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer, the OSHA 300 Log of recordable work-related injuries and illnesses did not have an entry of the total number of restricted or transferred days in the job transfer or restricted work days column.     On or about June 14, 2011, Ferrite International Company, Wadsworth, IL, did not enter the correct number of restricted  days for the following work-related injuries on the OSHA 300 Log or equivalent form for the calendar year 2009:    a) On or about July 27, 2009 an employee (#36-09) incurred a laceration on left thumb while grinding a steel plate. The injury resulted in restricted work activity for 10 days. The OSHA Log did not have a check mark entered in column I, and an entry of 10 days was not made in column L.    No abatement certification or documentation is required for this item.

On or about June 14, 2011, Ferrite International Company, Wadsworth, IL, did not enter the correct number of restricted  days for the following work-related injuries on the OSHA 300 Log or equivalent form for the calendar year 2008:    a) On or about July 31, 2008, an employee (#18-08) incurred a lumbar strain when bending over to pick up trays of parts. The injury resulted in restricted work activity for 5 days. The OSHA Log had the entry listed as 2 days of restricted work activity instead of 5 days.    b) On or about September 23, 2008, an employee (#35-08) incurred a laceration to the right hand while removing a nut from a press with a tool. The injury resulted in restricted work activity for 13 days. The OSHA Log had the entry listed as 3 days of restricted work activity instead of 13 days.    c) On or about October 22, 2008, an employee (#32) incurred a particle in the eye, resulting in 1 day of restricted work duty. The entry did not have a check mark in Column I, and an entry of 1 day was not made in column L.  No abatement certification or documentation is required for this item.
