29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access or were not secured:           a)  Madrigal, Oswaldo dba Valdo's Construction at 2250 Spruce Street, Boulder, CO 80302:  On or before 6/18/11, the employer did not ensure that a ladder used to gain access to the second floor porch had its side rails extended up 3 rungs or was secured.  Condition exposed employees to fall hazards of 12'9".     Abatement note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not  immediately marked in a manner that readily identifies them as defective, or were tagged with "Do Not Use" or similar language and withdrawn from service until repaired:          a)  Madrigal, Oswaldo dba Valdo's Construction at 2250 Spruce Street, Boulder, CO 80302:  On or before 6/18/11, the employer did not ensure that a ladder used to gain access to the second floor porch, and which included damaged rungs on the 1st, 2nd, and 4th steps, was tagged and removed from service.  Condition exposed employees to fall hazards of 12'9".     Abatement note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.404(b)(1)(ii):  All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which were not a part of the permanent wiring of the building or structure and which were in use by employees, did not have approved ground-fault circuit interrupters for personnel protection:      a)  Madrigal, Oswaldo dba Valdo's Construction at 2250 Spruce Street, Boulder, CO 80302:  On or before 6/18/11, the employer did not ensure that a working GFCI was on site.  The electrical circuits were used to power a 110 volt skilsaw that was not double insulated, and a 6 gallon Porter Cable air compressor.  Conditions exposed employees to electrical shock hazards. 29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, and enclosures was not permanent and continuous:         a)  Madrigal, Oswaldo dba Valdo's Construction at 2250 Spruce Street, Boulder, CO 80302:  On or before 6/18/11, the employer did not ensure that the following electrical circuits were grounded:         1)  One orange extension cord and one yellow three-way splitter cord were missing their ground plugs, and were used to power a 110 volt Skilsaw and a 110 volt 6 gallon Porter Cable air compressor;         2)  The plug to the skilsaw was missing its ground plug.  The skilsaw was not double insulated;         3)  The plug to the 6 gallon Porter Cable air compressor was missing its ground plug.       Conditions exposed employees to electrical shock hazards. 29 CFR 1926.405(a)(2)(ii)(I):  Flexible cords and cables were not protected from damage. Sharp corners and projections were not avoided. Flexible cords and cables passed through doorways or other pinch points, and protection was not provided to avoid damage:         a)  Madrigal, Oswaldo dba Valdo's Construction at 2250 Spruce Street, Boulder, CO 80302:  On or before 6/18/11, the employer did not ensure that extension cords were protected from damage.  An orange cord was laid across an access road, and was not protected.  Two other cords were tied in knots to secure connections, and had damaged insulation.  The cords were used to power a 110 volt skilsaw and a 110 volt 6 gallon Porter Cable air compressor.  Conditions exposed employees to electrical shock hazards.
