Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: na) On or about April 18, 2011 and at times prior A-1 Skyline Roofing did not ensure that employees were protected from fall hazards. Employees were involved with roofing activities without the use of any fall protection system. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
