29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On June 23, 2011 at a jobsite located at 5827 S. Cass Ave, Westmont IL. Employees were working on a roof performing masonry work. The employer did not provide adequate fall protection for employees. Employees were exposed to a fall of approximately 16.3 feet. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: On June 23, 2011 at a jobsite located at 5827 S. Cass Ave, Westmont IL. An employee voluntarily used a negative pressure respirator while wet cutting brick. The employer did not provide information to the employee contained in Appendix D. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
