29 CFR 1926.200(g)(2):  Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices:    a)  On or about June 3, 2011 and at times prior, Levi Contractors Inc. as the creating/controlling/exposing contractor did not ensure that workers and employees were protected from traffic hazards.    1.  No TCS (Traffic Control Supervisor) or other qualified person was on site to implement the TCP (Traffic Control Plan).    2.  Tapers into work areas were not properly erected in that they were too close to the work areas, were set up without proper spacing of devices and did not use enough devices.      3.  No signage was installed on side streets.    4.  No signage was used to signify lane closure or give flagman ahead notification.    5.  The flagger on site had received no training for this critical job.    6.  There was only one flagger used on a  two lane road, lane closure, with two intersecting streets,  on a sharp curve     7.  Devices were not used to designate the work areas as per the TCP.    8.  Employees did work in and walk in live lanes of traffic.
