29 CFR 1926.102(a)(1):  Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:         a)  Basquez, Maria at 3430 Uinta Street, Denver, CO 80238:  On or before 6/25/11, the employer did not provide safety glasses and require its use when the employees were shingling a roof with pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes.    Abatement note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:         a)  Basquez, Maria at 3430 Uinta Street, Denver, CO 80238:  On or before 6/25/11, the employer did not ensure that employees were using fall protection when shingling a two-story residence with a 5 in 12 pitch.    Condition exposed employees to fall hazards of up to 20 feet.    Abatement note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
